Guide Dog in Training Progress Report

Thank you for helping to raise, train, and socialize an OccuPaws’ puppy!

Please answer the questions below about your experience while working with an OccuPaws’ puppy.

This report will take approximately 30 minutes to complete. Please allot enough time to complete the entire report as you cannot save it and return later. If you close your browser window before completing the report all of your answers will be lost and you will need to start again from the beginning. Upon completion, your report will automatically be submitted to your coordinator.
